Example Request form – a form that does a reference interview

This is an example of a Request form that has been tailored by the library’s systems librarian for the specific purpose of accepting New book requests. The client submits the form in the same way, by following the on screen instructions to fill in as much detail as they can, then clicking on Submit (on this form you scroll further down to see the Submit button).
On this screen we have indicated some of the ways that the system administrator can design forms so they are easy for clients to fill out, no matter what type of question is to be submitted:

1. Each section on the form can be given a helpful name (in the purple box) together with instructions for filling out that section.
2. Each field included on the form can be names appropriately for its use in this specific form, and instructions specific to this field can be included.
3. All of the pull-down menu entries are tailorable through code tables set by your site administrator. They can be set up in the form with a default value showing, or with [Select option] showing. [Select option] encourages the client to select a value from the drop down list, but they do not have to select a value unless the field has been defined as mandatory (i.e. it has a tick next to it).
4. Default values can be set from drop down boxes and text entry boxes can have prompting information automatically included in them.
5. RefTracker provides many different types of fields for inclusion in forms, contact information fields, reference interview fields, and bibliographic files.
6. Tick marks indicate fields that the client MUST complete.


By using these features your library can have many different forms for many different purposes.
