Example Request form – very basic

This is an example of the very simplest sort of Request form where your System administrator is allowing the client to describe their requirements in free format.
To use this or any form the client simply fills out the data entry boxes, using the prompts provided on the screen.
The Question field (which may have other names in other Request forms) allows HTML formatted text to be entered – so clients can cut and paste formatted information from web pages into this field and any formatting (such as dot points and hyperlinks) will be retained. They can also use basic formatting functions like:
ctrl b for bold
ctrl i for italic
ctrl u for underscore
ctrl c for copy
ctrl v for paste
You can tell that a field is format enabled when you click in the field and a second border to the field displays.
When they are happy with the information they have provided, the client can simply click on Submit to send the request to your library. After clicking, the Submit button is greyed out indicating that the question is being submitted (and preventing duplicate questions from being inadvertently submitted).
The tick marks to the left of the data entry fields, indicate that entering information into the ticked field is mandatory.
If your RefTracker system supports more than one language, the alternative language names will be appearing on the line above the footer. Clicking the name of an alternative language will present that form in the alternative language, and all screens for the remainder of that client session will be in that selected language.
