How many Request forms does your service need?
We are about to learn how to create Request forms. But first we need to look at what forms your service will need so that you can apply what you learn to some real examples.
- Collect copies of all of the request forms currently being used in your organisation. You may have both hard copy forms and web forms in use.
- Check the statistics that you are already gathering. Are there any other processes being recorded there for which you will need to create request forms? What pieces of information need to be collected about these processes?
- Consider whether there are any other processes happening in your organisation that would benefit from being included in statistics, or that would benefit from the better control RefTracker provides e.g. things that come in via emails. Hand draw a form that includes all the questions required for a suitable reference interview for this type of request.
- Forms are used when follow up is required or when details of the service provided need to be recorded because the information provided was interesting, or because it will be used for other purposes (such as billing).
DeskStats should be used to record basic statistics when there is no follow up required, or any need for the information to be re-used.
However some processes do not require follow up, but require more than basic statistics – for example you may want to collect a number of attributes about training provided. Request forms can be used to efficiently collect detailed statistics. Are there any processes in your organisation that need detailed statistics gathered for them?
